NYSS CONSENT TO PARTICIPATE/ASSUMPTION OF RISK
1. ASSUMPTION OF RISK AND RELEASE: As parent(s) or legal guardian(s) of
the participating youth (the “Participant”), a minor child, I consent to the Participant’s
participation in the sailing program of the Northport Youth Sailing School (“NYSS”), a
not-for-profit corporation organized under the laws of the State of Michigan. I
understand and agree that, even though NYSS sailing classes and related activities are
supervised by certified sailing instructors, and despite reasonable precautions, there are
still dangers and risks inherent to sailing and the NYSS sailing program, including injury
to or death of participants. I, both individually and as parent(s) and/or guardian(s) of the
above named minor, do release, discharge and indemnify NYSS, its employees,
officers, teachers, volunteers, sponsors, donors, assistants and agents from any and all
damages, claims, liabilities, demands, and causes of action, known or unknown,
because of injury to or death of the above minor as a result of the above Participant’s
participation in the NYSS sailing program, whether or not caused by the negligence of
NYSS or its employees, assistants or other participants.
2. CDC PARENT ATHLETE CONCUSSION INFORMATION FORM: I will carefully
review the risks described in the CDC’s two-page Parent/Athlete “Concussion
Information Form.” I understand that I must review this form with the Participant, and
that both the Participant and parent/guardian must sign the form and return it to NYSS
either by uploading it through the Active Networks registration portal (strongly preferred)
or by bringing a physical copy of the form to the first day of class.
3. MEDIA RELEASE: I understand that participants are sometimes photographed
and/or videotaped while attending NYSS, and that these images may appear on the
NYSS website, and may also be reprinted by other groups (typically
organizations/companies such as the Leelanau Township Community Foundation that
provide financial support to NYSS).
